Sports Physicals

When: Tuesday September 7, 2010

Time: 5:30-7:30
Cost: $15

NOTE: All athletes participating in Fall Sports (Volleyball and Soccer) must have a valid sports physical turned in by September 8 or they will not be allowed to practice or play.  

Forms Needed

All athletes interested in playing LCS athletics must have the following forms completed and turned in before they are allowed to participate in an athletic game.

1. Sports Physical Form (download) (Please note that LCS uses a specific physical form)
2. Putting Christ in the Contest Form (download)
3. Student Athlete Contact Information Form (download)
