Sports Physicals
When: Wednesday September 16, 2009 Where: Legacy Christian Middle School Time: 6:30-9:00 PM Cost: $10
NOTE: All athletes participating in Fall Sports (Volleyball and Soccer) must have a valid sports physical turned in by September 17 or they will not be allowed to practice or play. |
Forms Needed
All athletes interested in playing LCS athletics must have the following forms completed and turned in before they are allowed to participate in an athletic game.

Athletic Involvement
Athletes that participate in athletics are an important and necessary part of a team. Regular attendance at practices and games is important for the student athlete. Faithful attendance shows the athlete’s commitment to the team, allows for growth through the best possible conditioning and allows the athlete to receive all instructions, announcements and available training from their coaches.
Excused absences from practices and games include:
• Times when the student athlete is ill. o Please note that a student must attend 6th, 7th and 8th hours to be considered eligible to participate in a sports contest on that day. • Medical appointments that can not be made for other times or days. • Other family situations that are communicated in advance with the coach or athletic director. • Involvement with other Legacy Christian School programs. (musical, committee meetings, track/softball/baseball, spelling bees or other related topics) (School related but not excused: Ski Club, SCHS athletics )
Student athletes that have an unexcused absence from a practice or game will:
• Not be allowed to play in the next available game. Unexcused is defined as not letting the coach and/or athletic director know that they will not be at a team event. This applies even if the absence falls under the excused criteria.

• Student athletes that leave a team practices early three times during the season will not be allowed to play in the next contest. This consequence will again occur after the 6th and 9th absence.
• Student athletes are not allowed to leave a sports contest in the middle of the contest. If this occurs it will be considered an unexcused absence. |
